How to put selected check box label (from a previously selected multiple select question) into the subsequent text question?

Dear @Kal_Lam

I was wandering how to send the value label to the concerned check box. Please find the attached screen shot and the kobo tool for your kind information.

Basically, what happens is when I select topmost item from the group of list box, it goes to the first text box but when i de-uncheck and check second Item it is seen in the first check box which was supposed to be in second assigned checkbox.
