Hello, @Kal_Lam thanks for the suggestions. I have done the validation several times. There is no problem at all. Recently, my Organization has shifted to a cloud based server and I am suspecting if that causes the problem.

Any further thought please?

@AsratB, so you mean you have a self-hosted KoboToolbox server from where you are trying to deploy your survey form? Have you tried deploying it through one of our public servers?
