Often a 500 error while deploying is due to an error in form logic somewhere.

There is an excellent online validator that should help identify any problems with your XLSForm - if you upload your XLSForm there it will tell you what some of the potential issues might be.

Hi @maggib, I found the duplicate that i have highlighted with yellow and marked the font with red within list_name lf3sx85 whose name prefer_not_to_answer is seen repeated twice within list_name lf3sx85. All you need to do is go to your choice sheet and then check row 108 to check for duplicates in your choices sheet.

BTW, feel free to validate your XLSForm through this online validator to see if there are any syntax issues in your form that are affecting the deployment.
