I have 5 entries in a form and I have created a vertical custom form to display the same.
However I do not see any data in form of a graph,

@satyadeep, it seems like you are on a self-hosted server. Maybe you will need to contact your sysadmin to look at this issue and see if there are any installation issues. As a quick check, you could also make some dummy entries to one of our public servers and see how it behaves. If it behaves differently than what you see on our server, please contact your sysadmin for further support regarding the installation issue.
