You can validate your form by using the ODK - XLSForm Online . But also you may want to check choices sheet in your form because apparently one of the choices uses the same XML value so either you need to chance it or allow_choice_duplicate. You can find the duplication with Validator or download your XLS form, go to choice sheet choose the “name” column, and go to home tab and choose conditional formatting and highlight duplicating values, so you need to change one of the duplicating values to get rid of this error
