I want to add scores to my questions

Hello,

I have a set of questions which I need to design and add scores then total scores as well. For example:
What stock management system is used for a hospital?
No System…1
Paper Based…2
Electronic based…3
The scores are 0 for no system, 5 points for paper based system and 10 points for digital system…

Please help
