I would like to know how to insert a cover page before starting to write the questions?
@pramde1977, you could always use the note question type at the beginning for this. Many users use the note question type to keep the consent form at the beginning.

Hi @pramde1977, There is a type of question “Note”, this question type is useful for this kind of informative messages in the begging of the form or any point you would like to inform the user, this question type doesn’t take any input. You can add this question to your form and later you can download your form and edit the text as you would like to, when you add it first time though the web interface it will be like one line of text but you can download your form and edit the style of these note then re-deploy your form.
If you would like to dedicate a specific page as Cover page. You can use multiple page style. You can find more information below
