Indexed repeat for select-multiple followed by text

I am trying to create an indexed repeat where the selected multiple choice answers appear as the subjects of several text boxes where participants can provide further details to each multiple choice selection (see image). I have tried to build this in XLS using a method from another post but have not been able to get the indexed repeat labels to appear properly in the form. I cannot upload the XLS form as a new user unfortunately.
