All, I am adding a 4 level cascade list into my online formbuilder to include zone, region, district and health facility. The first three work, but the options for health facility don’t load into the form builder, it just says option 1 and 2? Would anyone be willing to look at my template and see if they notice any errors? I feel like I’ve checked for all the major ones, but must be overlooking something.
