@Kal_Lam with the construction of questions, is it possible to have some of them in tables and will these appear as tables on a tablet. If yes this could help me reduce the number of questions. Attached is an image of a table question.
@theo27, you should be able to design the same using the matrix question as outlined in the support article Question Matrix Response Type. However, the data entered in the matrix question does not stay in the exact format as seen in the question.

@Kal_Lam Alright then i guess i have to maintain the above table question in a sentence form. Since the targeted people to fill the form will be using either a tablet or a phone.
