Is there a way matrix can allow for more than 3 columns for proper display (though from some searches done so far, I noticed max. 3 columns were used. (see attached, as the fourth column which is VERIFICATION eventually displayed differently as a row instead as column)

Regarding your issue, you could always play with the grids as outlined in this Enketo Site. Or you could design your survey form through the form builder UI as outlined in the support article Question Matrix Response Type.
