Is there any calculation that can give nutritional status of a child if we put weight and height of a under 5 y child

I want my data collectors to enter weight and height of a child (12 to 59 months) and they could see the nutritional status of a child whether a child is SAM or MAM or Normal, using WHO charts or any calculation approved by UNICEF/WHO/World Bank which has the authenticity. Please help.
