Despite that, 4 forms out of 29 are missing the signature. So, yes, when we need to collect signatures we are literally taking a photograph of a piece of paper with the signature on it. In this form we just use an actual photo + signature - 2 images. However, Form B seems like a nice workaround as our enumerators use Android tablets. It’s a shame it’s not working with iOS.
With a different form, I also noticed that a similar error also occurs with repeat_groups with image questions.
With this, there seems to be conflict when handling several images (or attachments, as you managed to discover), regardless of whether there’s more than one image question, or a repeat_group with more than one nested records (images).
There is a known issue with multiple signatures currently getting lost in Enketo. This should be fixed in the upcoming imminent Enketo 7.5.1 release. Thanks for your patience on this.
The issue of signatures missing and, in certain other circumstances, images in general being ‘lost’ should now both be fixed in the new release just now.

I would like to confirm whether this release will impact on only the newly collected data or it also resolve missing signatures of the previously collected data as well?
Both fixes that went into this release that potentially affecting signatures (one in Enketo, the other in Kobo) will only affect newly acquired data.
If you think you are missing submission critical data then it might be possible to recover them, but it will depending on what exactly caused them to go missing, and when. Attn @Kal_Lam

Thank you for your response. I have tested collecting some new data entry with a questionnaire that consist of two signature questions. It is working fine now; I will continue to monitor and updated here in case there is any error.
By the way, I would like to learn more about the “recovery”. What are the possible approaches I should try?
