Kob toolbox and ODK table questionnaire

Hi, can anyone help me to how i change this table to kobo toolbox odk survey questionnaire

if some one tick or select the options (available , not available ) can also add a comment the comment isn’t Mandatory but select option is.

if I use matrix question type that did not work odk.

@Khayre, the matrix question is only compatible with Enketo aka the web form. It’s not supported with the Collect Android App.
