Loading Error: Preview

I used an XLS table for cascading-select, but when I preview the questions the following error message displays:

Extra content at the end of the document

I tried to validate the form using the online validator but the validator did not accept the XLS file. Could someone please help?

Can you share the outcome from the online validator? and if possible xls form?

@gnarula, an XLSForm should always have at least these three sheets:

survey tab (mandatory)
choices tab (optional based on the questions that are present in the survey tab)
settings tab (optional)
