Logo image not appearing on form

I have created forms and added a logo for the forms but it appears broken.

Please assist on how to fix this

@Elyjoy, could you try underscore _ instead of blanks both in your file and in your form?

Instead of RFHA inc logo.png could you try RFHA_inc_logo.png ?
