My form is collecting data bt in the backend columns are blank once submitted, what might be the issue

hi everyone. i have built a form that is to be used next monday for data collection. in the app, the fform does calculation nicely and flows nicely as you fill and finally finalize and send. in the backend , the columns are empty. if u fill in the web version, the data is collected nicely and is there. so the rows filled in the app are empty. i am attaching my xls form,

Its not answering my issue, the new child row is being created in the repeat sheet but the responses in subsequent columns are missing.
