Pressing "+" only replicates one group of questions on my form instead of entire form

Hello,

I am creating a survey form and ran into a strange issue with data entry. There are three groups of questions: demographics, test scores, and finally a short confidence survey. I would like it so that when the groups are completed the person entering data can hit the “+” at the bottom and another form appears beneath the completed form for a new student. Unfortunately the only thing that replicates when I press this is the last group, the confidence test. Why could this be?

i did notice that the confidence test group of questions has a blue tone to the background. Is this part of the issue? Thanks for any help you can provide.
