@aprcglobal, I have split your post so that we could document our issues systematically. Your survey project should have a question (either a select_one gh48h45 or select_multiple gh48h45) in your survey sheet, but at the same time, gh48h45 is missing under the choices sheet. You could check this out by downloading your survey form as an xlsform and then doing a search by pressing CTRL+F under the survey tab as well as the choices tab. BTW, you could always validate your XLSForm through this online validator? The online validator should help you identify any syntax issues that are affecting the form deployment.

@okware_256, all you need to do is follow the steps outlined below: Open online validator. This screen appears as shared below: [Image 1] There press Choose File and upload your XLSForm.
