Hi, I had the same kind of error, while creating the form, i switch account then i forgot to re-enter the account that i log out.

@jihadsh, if you see such issues please download your survey form as an XLSForm so that you will not lose the latest version of the form that is in the UI. Feel free to reach us if the issue still persists.
