How can i generate (give) every survey a unique QR cod?

Or it contains some beneficiary information, such as name and ID number, for example
@salaheddinieyyubi, so you mean you wish to create a QR/barcode from the details that you have in your project (like using the name, age, sex, etc)? Correct me if I misunderstood you.
