How can I use the randomize function to change the order of options, keeping the last option fixed?

Short answer: no. The randomize() function will randomize all the select options; there is no way to exclude a particular one. Could you perhaps rephrase your question(s) to first ask “Can you answer such-n-such?”, and then only show the (randomized) select list if they answer yes?
