Rename the Photos as the number of entered questionnaire

I would like to know is there any way that can take the name of the taken photo with the Questionnaire number

Q1 Type-integer Q1 Numnber of the questionnaire
Q2 Type-Image Q2 take a photo of ID

I need the outputs of Photos will be carrying the entered Q1 number to have easy access
