Repeat questions for options selected

I am trying to develop questions, where if I select 1, then a set of questions appear for one, and if I select 2, then same questions appear twice.
For Example:
If I click Option 1, indicating one child: Name, Age, Sex, Name of school appears once
If I click Option 2 indicating two children: the same questions above need to appear twice for each of the two.
I just need the set of questions to be duplicated depending on the options selected.

I would appreciate your help.
