Server Error (500) seen while trying to deploy a form

Dear sir/mardam

I tried to deploy me xls from, but I got the message " Unable to deploy. if this problem persists, contact help@kobotoolbox.org. Server Error (500).

Could provide me some tip to solve this problem please?
