Hi John,
The surveyor needs a username to fill out the form with Kobo Collect. They can either create a Kobo account and you share your form with that username, or if you already have data entry credentials, share those with the surveyor to set up the app. Alternatively, they can use web forms if they prefer not to use the Collect app.
