Some data field in my exports are duplicated and returns empty

Hello, Mr. Lam. Please assist me. I validated the questions in a questionnaire on Kobotoolbox, and after testing and downloading, some data fields are not returning the values I submitted. I am a PhD student currently working on my project.

I tried exporting data in a different format to see but still the issue persists. pls help.

I am willing to share the entire questionnaire and the exports. FIND ATTACHED
