some questions are missing when I am trying to preview my form in form builder
Did you check your form with the Online Validator? Make sure that no labels are missing.

@wkammera, could you share a screenshot of how it looks when the questions are missing? In addition, have you validated your XLSForm through this online validator to see if there are any issues causing this?
