Table creation in Kobotoolbox

I created this table in SurveyCTO with some index formulas and the table looked exactly as I wanted. See attached

I need help creating the same table in Kobo. I have tried the steps in the Question Matrix Response Type article but still unable to create the matrix table. I have uploaded the xlsform for help as I may be doing something wrong.
