Welcome to the community, @admin1! Maybe you should download your survey project as xlsform and then validate them through this online validator. The online validator should be able to help you identify syntax issues that are present within your xlsform.
Looking at your error message I presume that your survey project should have some names duplicated. The names from both the survey tab as well as the choices tab should be unique and should not be duplicate.
