Let’s tackle your problems one by one,
1- You need to rename your sheets to survey, choices and settings. Right now, your sheets are named as choices sheet and settings sheet, removing the sheet from them will solve this.
2- In the choices sheet, you need to rename your list_names to location and relevance.
It’s a good practice to validate your form as @osmanburcu said in here: ODK - XLSForm Online so you can see any syntax issues that may occur when you are developing a form.
