someone shared a form with my account
how I can deactivate this shared form from my account
and don’t appear in my page

But as for now, if this is causing you an issue please reach us at support@kobotoolbox.org through your email that was registered with your user account that has this issue. Also please provide us additional details such as:
- Username (yours)
- Username (that the unknown user shared his/her project)
- Project name
- Server
We shall un-share the project for you.
