Looking at your xlsform i have slightly modified your xlsform as follows:
In the survey tab of your xlsform
In the choices tab of your xlsform
In the settings tab of your xlsform
Data entry screen as seen in Enekto:
Please note that the GIF file at the moment is not supported in KoBoToolbox while the png image is supported (as shown in the image above). Also please note that it may take a moment to load the image in the data entry form depending upon the size of the image that has been uploaded in the server.
