Am getting an error when i try to load the form of the project which i have deployed please explain to me in a very layman language how this can be solved

hello @Kal_Lam thanks for the feedback , you see the trouble am facing is that , i have created a new project and i created some question, then i save it then i deploy and status says its deployed it but when i want to preview or fill the form from the generated enketo urls i get the above error thats my main problem
