Can't load form in the App

Hi Kobo team,

I’m trying to load one of our forms in the app. I was able to download the form. But when I tried to open it and log information, this error message popped up:

Kobo error
Kobo error1080×2400 122 KB

My teammate tested it and experienced the same error. We both have the latest version of the app.

Could you please advise on how to solve it? Thank you!

Welcome back to the community, @yulanlu! Is the tree_species_table a CSV file that you have included with your project? If so, it would be beneficial to review its format to ensure it is well-structured.
