Cascade selection does not work

Hello community, greetings.

I have a problem with the cascading options of one of my forms that I have not been able to solve. It works well for two levels (state-municipality) but I cannot make it work for three levels (country-state-municipality), simply selecting “Country” does not display the options for “State” and neither for “Municipality”.
This is how cascading options are created in Excel:

This is what results when I open the form:

As can be seen, despite having selected Country, the other options (state-municipality) do not show any of the options that are expected.

As much as I have gone through it and tried it in many ways, I cannot make it work.

Very grateful for the prompt reply. Unfortunately I have not been able to fix my problem with cascading options. I send the excel document to see if they can tell me where the error is …

Did you remove the survey sheet of your XLS form?

No, the survey sheet is in the same file, the one I sent in this post I extracted for display purposes
