I want to add a cascade choice question in my survey. I am using formbuilder for the same. I am not able to shake off this error. Please help me out

I have tried following the steps. However, the same issue persists. I have tried to identify the rows which is causing this issue. The issue arises from the rows 76 to 84. I am unable to identify what exactly is the issue in those rows.
Please help me out. I am attaching the file here for your reference
