Cascading questions

Hi Kobo community,
I have a challenge in adding cascading questions in my project, every time i copy from excel and paste, only one question out of the 3 questions appear. What should I do

I am trying to upload my xls form but i keep getting message sorry new users cant upload attachments and when i tried copy and paste, the form does not come out.
However, I tried to reply using my email address, I don’t know if you received it.
