I created my form using the online form builder. I deployed it once, then made some edits and tried to upload/deploy the updated version. However, every time I try to deploy the changes, I get an error message.
The screenshot shows the message:
“Unable to deploy… On the ‘choices’ sheet, the ‘name’ value is invalid. Choice names must be unique for each choice list.”

Pleass have a look at this documentation:
To fix this:
Download your form as an XLS file
Open the choices sheet
Check the name column for any duplicates
If those duplicate names are intentional and required for your form design, you can allow them by configuring the allow_choice_duplicates setting in the settings sheet.
Thank you, i did as instructed but here is another problem.
No preview, form keeps loading forever. Lower left i see ‘select_from_list_name’ but know really what this mean and how to fix it.

It will show you the exact line and column where the problem is happening and suggest how to fix it.
