Choice Name Duplicate Error

To fix this:

1. Download your form as an XLS file

2. Open the choices sheet

3. Check the name column for any duplicates

If those duplicate names are intentional and required for your form design, you can allow them by configuring the allow_choice_duplicates setting in the settings sheet.
