Choices missing when adding an external select file

I got survey that pulls data from external sheet from single choice but problem is ,when i add external file and save the From it shows code in red and choice missing

Would you mind sharing both the files (xlsform as well as csv file) so that we could have a closer look at your case. Maybe you could share only the section that has issues.
