When I go into Kobotool box to view form submissions under ‘Data’, the date reflected there versus the date actually submitted in the form. Please see the examples below:
@cocoroach17, could you kindly let us know the server you are using? Is this an issue with only a single submission or is it an issue with multiple submission?
How is the date entered? If you use a today() calculation, it will always update on edit/open/save. Also the local device date/time/timezone might be different from the server date/time/timezone.

I am unsure what you mean by a short extract.
An example/extract reduced to a minimum of questions and choices, in XLSForm format, please, to allow the community to focalise your issue. The example/extract should also pass the Online validator. without errors,

34 date Individual_Care_Plan_Development_Date is the field giving me issues.
