Welcome back to the community @george. It seems like you have media files in your project. You should have uploaded them but failed to redeploy your project later. Could you redeploy your project? That should solve your issue.
It has been appearing like this when we try to access the tool. from my MEL teammates and my IT end for two weeks.
Kindly find the error link below -when we open the form shows this,for the last two weeks.
