You should follow the instructions carefully on how to set up the form as indicated in the following article

When I look at it, you are defining your csv using a different name distritosS yet your CVS you uploaded has a different name.

pulldata(‘distritosS’,'codigo’,‘distritos_sof’,${what2})

I have 2 different csv files ,the one that i uploaded in the forum is the one whose having issues on kobo.

I did not see a common match that the pulldata function requires in the files (xlsform and csv file) you shared.

In the survey tab of your xlsfrom:

In the choices tab of your xlsfrom:

In the csv file:

Try fixing it and you should be able to get the information from the csv file.
