This is interesting, because it shows no errors in ODK-Validate (ODK - XLSForm Online) and when I upload your xls form it works as intended:
Did you by any chance edit your form and forget to redeploy it?
It seems you used / in variable or choice list names. This is not allowed, see naming rules for ODK/Kobo/XLSForm.
