Form is not working on android app but works on Browser

My form works perfectly over browser…
But doesnt work completely on Kobo collect app

When i select ADD NEW, new options shud comes in…
It does happens in browser, but not on Kobo Collect

Maybe you will need to use the numbers instead of using the - and spaces under the name column of your CSV file:
