Form uploaded on the server but cannot find it on the tablet

I have uploaded my xls form on the server, it has been deployed by if i log onto my tablet, I cannot see any forms (details in the picture) . please assist. the url is correct, server name correct also

Welcome back to the community, @anjali! If your server configuration is correct in the app, could you ensure you have logged into your KoboToolbox account at least once (ever)? Please note that if you have never logged into your account, you cannot get the blank forms to your app even if the project has been shared with your account. Please update if this worked for you.
