@ncne2022, as advised by @hakan_cetinkaya, you will need to check for duplicate name from your choices sheet. I see that your list_name lga has duplicate values for 1, 2, 3, … These name should be unique for all the ones you define under lga. You should be able to learn more from our previous post:
