Your XLSForm format is incorrect. The “type” column should only contain question types such as “text”, “decimal”, “integer”, “select_one listofqno”, “select_multiple listofmul”, etc.
In your form, some type keywords are incorrect because they start with a capital letter. Please refer to the following resources for correct formatting:
