How to design a survey form with a likert appearance?

Hi @PALASH9698,

You could use the likert from the appearance to display your response options as outlined below:

If you are using the form builder UI:

Design a normal select_one question type as shown in the image below:

Select likert from the Appearance (Advanced) as shown in the image below:

If you are using the xlsform:

In the survey tab of your xlsform:

In the choices tab of your xlsform:

Data entry screen as seen in Enketo:
